Terms and Conditions for Midway Clinic Mobile Health Service (PHONE CONSULT NO: 0594060678)

Thank you for using our mobile health (mhealth) site. By using our mhealth site, you are agreeing to these terms. Please read them carefully.

IMPORTANT NOTE: This mHealth site is not to be used to address emergency or life-threatening medical conditions. If this is an emergency, please report to our physical facility immediately.

PLEASE READ THIS AGREEMENT CAREFULLY. THIS AGREEMENT GOVERNS YOUR USE OF OUR MOBILE HEALTH SERVICE. IF YOU DO NOT AGREE TO THE TERMS OF THIS AGREEMENT OR ANY REVISED VERSION OF THIS AGREEMENT, PLEASE DO NOT USE THIS SERVICE.

1. NATURE OF OUR SERVICE

Our mHealth site is not intended to completely replace a full medical evaluation or an in-person visit with a physician.

This site is only for General Medical Consultations and NOT specialist or Antenatal services.

In an effort to make primary healthcare easily available and to reduce our waiting times, we have rolled out this service and hope to provide you with quality affordable primary health care at your convenience.

This phone consultation service is available all week from between the hours of 1pm in the afternoon to 7am the next morning. No mHealth service is available from 7am in the morning to 1pm in the afternoon.

This service is currently not available to our insurance clients (both national and private health). However, it is available on-cash basis and for clients from companies registered in a contractual agreement with the Clinic.

This service gives provision for laboratory samples to taken be taken within the confines of your home or office. Off-site samples are taken between the hours of 8:30am and 4:00pm.

This service currently does not run deliveries for medications.

2. PATIENT’S RESPONSIBILITY

It is your responsibility as the patient to provide true, sufficient, accurate and up-to-date information about your symptoms to your assigned doctor. Incomplete information may lead to false evaluation that the doctor cannot be made responsible for.

You acknowledge that you understand that in rare cases, a lack of access to reliable and complete medical examination may result in adverse drug interactions or allergic reactions or other judgment errors and agree that you remain responsible for the provision of all relevant information. In some cases, the doctor may determine that your condition requires a face-to-face meeting.

3. OUR PROCESS FOR USING THE MOBILE HEALTH SERVICE

1. Starting the process

1.1. Call our registered mHealth number: 0594060678

1.2. You will be asked for your folder number or personal details to enable us find your health records.

1.3. A message and a link to our terms and conditions will then be sent to you.

1.4. By continuing with mobile money payment it is assumed that you have read and agree with the terms and conditions herein stated.

2. Phone Consultation Service

2.1. Upon receipt of payment, the doctor will check your medical records and you will be called back by the doctor within thirty (30) minutes for a consultation session.

2.2. The phone consultation is expected to last between five (5) minutes to ten (10) minutes.

2.3. Depending on your condition, the doctor may:

2.3.1. Offer suggestions or counselling or,

2.3.2. Recommend laboratory investigations or,

2.3.3. Prescribe medication.

3.0 Laboratory investigations

3.1 If laboratory investigations are requested, you may request to have the samples taken at your home or office or,

3.2 You may walk-in to the clinic to do them or,

3.3 You may ask for the requested investigations to be sent to you by Email or Whatsapp if you prefer to have them done at any near-by facility and upload them onto our platform for a doctor to view.

4.0 Prescriptions

4.1 Prescribed medication may be obtained from our facility or nearby facilities at your convenience.

4. PAYMENT OF FEES & BILLING

Payment is to be made in advance of consultation.

You will not be charged for a review of your requested labs from your consultation. However, any further queries will require a full payment before access to a doctor.

Mobile money payment is made payable to Merchant number 0241929222 with registration Midway Clinic Ltd.

Charges for laboratory investigations remain the same as when done within the facility however, there would be a service charge for transportation depending on your location.

5. CHANGES TO THESE TERMS and CONDITIONS

We may change these Terms and Conditions at any time, as we reasonably deem appropriate. Upon any change in these Terms and Conditions, we will post the amended agreement for your perusal before utilising the service. Your continued use of the service following such notification shall constitute your affirmative acknowledgement of the Terms and Conditions, the modification and agreement to abide and be bound by the Terms and Conditions, as amended. If at any time you choose not to accept these Terms and Conditions, including following receipt of notification of any modifications hereto, then please do not use our mHealth platform.
